Q. What is the admissions process?
A. Choosing a place to call home is one of life’s most important decisions. Our dedicated admissions staff will answer your questions and help you navigate through the admission’s process.
Bordeaux Long-Term Care has an admitting nurse liaison who visits prospective residents at their home or in the hospital. The nurse liaison makes an initial assessment of the patient’s medical situation to evaluate if he/she meets the required criteria for nursing home admission and determine the level of care needed.

Q. Does Bordeaux Long-Term Care accept Medicare?
A. Bordeaux Long-Term Care accepts Medicare and Medicaid; private insurance including: Blue Cross, Aetna, Cigna, HealthSpring, Windsor, AmericaChoice, AmeriGroup,Veteran’s Administration and private pay. Our social workers will be glad to assist you in determining the availability of third party coverage.
Q. Are there restrictions on the type of residents admitted?
A. Bordeaux Long-Term Care’s medical and nursing professionals are trained to provide various levels of care for those individuals who meet clinical criteria for admission. Our staff is fully-equipped to care for the variety of individuals admitted to our facility, including residents with rehabilitative needs relating to spinal cord injuries, head injuries and cerebrovascular accidents as well as those facing health conditions associated with the aging process. We also provide a secured unit for residents who suffer from dementia or Alzheimer's disease.
Q. What services are included in the basic daily rate?
A. Basic daily services include nursing care, meals and nutritional services, therapeutic recreational activities, social services, restorative care, laundry services, personal shopper service, beauty/barber shop and free basic cable television programming.
A. Residents and/or third party payors will receive separate bills for physician and nurse practitioner visits, clinic visits and ambulance services. Phone service is also the responsibility of the resident or family.
Q. What types of menus are available for residents?
A. A five-week menu cycle, with offerings prepared by our resident chef, is provided for all diet requirements and restrictions. Dietitians have alternate offerings for residents who have other preferences. Menus are posted weekly in the Day Rooms where many residents congregate to enjoy the food and company. Residents who have supplemental nutritional needs have approved items added to their daily menu. Ice cream, soft drinks and juices are available on the floors for resident snacks, diet permitting.
We provide special holiday menus and our Resident Council selects one special “menu for the day” each month. BLTC also offers Crawford’s Café, which is a unique monthly fine dining experience.
Q. What are the skills and credentials of the nursing staff?
A. The intermediate care nursing unit is staffed by certified nursing assistants and licensed practical nurses. Each floor is supervised by a patient care manager (PCM) who is a registered nurse responsible for the care and services provided on the respective unit. Our skilled units are staffed with registered nurses to care for those with more challenging medical needs. Charge nurses are available on evening, night and weekend shifts and a registered nurse serves as house supervisor to address changes in a resident’s condition or emergency situations.
Q. How often do physicians visit the facility?
A. The medical director and staff are available Monday through Friday. Nurse practitioners and physicians are on-call for critical situations 24 hours a day, seven days a week.
Q. What is included in BLTC’s rehabilitation therapy program?
A. Your rehabilitation plan of care depends on the level of therapy you need to reach your full potential. We will work with you to regain independence and improve:
• Self care skills: feeding, grooming, bathing and dressing
• Mobility skills: transferring, walking or self-propelling a wheelchair
• Communication skills: improving language skills
• Cognitive skills: improving memory and problem solving skills
• Social skills: interaction techniques
Q. How can I schedule an appointment with my loved one’s physician?
A. Appointments with the medical director or attending physicians can be scheduled by calling the medical director’s office at 615-862-6877.
Q. How can I participate in my loved one’s care?
A. We encourage you to visit as often as possible to join your loved one for mealtime or attend one of our many special activities. We have a gathering room with vending machines, tables and chairs that give you another convenient meeting location. You will be invited to participate in a quarterly care plan conference. If there is a change in your loved one’s condition, we will schedule more frequent meetings. We also encourage you to attend resident/family social events. A schedule of monthly activities is listed in the recreation calendars distributed to residents and available at the Information Desk. Bordeaux Long-Term Care also has a Resident Advocate who is available to discuss issues with residents, families or responsible parties.
Q. What room accommodations are available?
A. Resident rooms are either semi-private or private and they all contain sprinkler systems for fire safety. Standard accommodations in each room include a bed, bedside table, elevated tray table, wall hung television stand with basic cable access, closet, hand sink, bulletin board and a connecting bathroom. Our central HVAC system keeps resident rooms at comfortable temperatures throughout the seasons. Families wishing to add furniture or small refrigerators may obtain recommended dimensions from the Facilities Management Department.
Q. Are family pets permitted to visit?
A. Family pets are permitted to visit after the Volunteer Office and Information Desk receive a veterinarian’s statement indicating the animal’s vaccinations are current, the animal is in good health and has a pleasant demeanor. Family members who bring an animal into BLTC are asked to use the main entrance and provide veterinary records to Information Desk personnel. The recreation room is also available for pet visits.
A. BLTC is a smoke free facility. Supervised smoking is allowed in an outdoor designated area if permitted by the attending physician. Smoking materials are maintained in a locked area and will be given to residents at designated smoking times. Family members are asked to check-in all smoking materials at the Information Desk. Failure to comply with the smoking policy may result in discharge from the facility due to safety concerns.
Q. Can I bring my loved ones personal items?
A. Families are encouraged to bring pictures, photo albums and other items that may help the resident feel at home. Buying expensive items is not recommended.
Q. Is long-term acute care available?
A. Kindred Hospital Nashville leases one floor in the Birmingham Building providing long-term acute care for critically ill patients in need of extended hospitalization.
